How to Use EmailInsert Template Pop-Up for my own created custom SMS Activity for inserting templates

I want to use Email insert template pop-up in my own created custom SMS Activity.

so that I can add template by pop-up in my SMSdescription field.

Please guide me. how can i achieve this things.
