Created new copy of opportunity automatically

Hello All,

I am creating opportunity in CRM i found a same copy of opportunity in CRM it is automatically created.

Its not creating every time,randomly i found this for few records.

There is no any workflow or plugin is triggers from here,its strange how it created.

Please suggest they to away this issue.
