qualifying reactivated lead creates duplicate contact

Hi,

   After reactivating lead and then qualifying it again creates a new contact i.e. duplicate to contact created on first qualifying of that leads

Problem is that when tracking contact with email from CRM App for outlook  it shows to create new contact and doesn't track it .

which can be found out while searching it manually in outlook,

What should i do to track the first created contact only and prevent from creating duplicate contacts and duplicate detection is checked for Contact Entity
