PhoneCall CallFrom/To Field not getting auto populated in crm 2016

Hi,

We have migrated our application from CRM 2011 to CRM 2016.

As in CRM 2011 "information form" was main form so in 2016 it created two main forms.One is information form(our customized form) and another is PhoneCall form(out of the box form).

The field Call From or Call To is not getting auto populated post migration for outgoing or incoming direction when we are creating new phone call for Information form but where as it is working in out of the box form.

Any idea what could be the reason for this?
