CRM 2016 Cache and Cookies

We just upgraded to CRM 2016 not an inplace upgrade. After couple of weeks, its been giving us an error  "Access Denied" and "hash value key has expired" even though the users has the correct security permission; users are not able to open opportunities, upload attachments, download attachments. The work around we have been doing is clearing cache and cookies or using different browsers.

Not sure why this is happening, we already installed CRM 2016 SP1. Could this be a permission issue on the CRM Service? Asynchronous issue? We never had this issue in CRM 2015 or CRM 2013.

Thoughts?
