Append dropdown value to notes title in social pane

Hi experts,

I have a requirement where the user chooses a value from a optionset field and then fills in the note text in the social pane.

Now, this value from the optionset is appended to the title of the note.

Is this possible in Dynamics? I'm assuming this is not possible OOB.
