SSRS Audit Report For CRM 2011 Custom Form

Dear Friends,
We've a Microsoft Dynamics CRM 2011 in our company.
I want to make a SSRS report for a custom form.
I want to follow the audit history in that SSRS report. 
In this SSRS report, I want to keep track of changes for old records which is created from that custom form.
I would ask for help from friends, who prepared a SSRS report for this style.
