I have seen very strange behaviour (which I never came across) today in CRM 2013.
I have one lookup field, when I open it first row is auto selected which the default behaviour of CRM but the ADD button at the bottom is disabled, if I deselect the record and select it again ADD button gets enabled. Please find screenshot...
